This year’s conference theme, Transforming Healthcare at Home: Solutions for Population Health Management, Care Transitions, and Gaps in Care, featured exhibitors, speakers and breakouts including a session lead by Ann Spenard, Qualidigm principal and vice president, on the home health version of INTERACT®, a quality improvement program that focuses on the management of acute change in patient condition.
