The Chronic Illness and Disability Payment System (CDPS) is a diagnostic- and pharmacy-based risk adjustment model and classification system that supports health status-based capitated payments for Medicaid and Medicare populations. CDPS was first developed in 2000 by the University of California, San Diego (UCSD), and has been updated and enhanced several times since then. Currently, CDPS analyzes and stratifies populations across 58 risk categories based on disease type or body system, as well as 15 pharmacotherapy-based conditions. In 2007, Altruista Health partnered with UCSD to integrate CDPS into the GuidingCare Care Management platform.
