SPAC international provides ONC HIT certified Chronic Care Management Physician, Patient and Pharmacy Portals for physicians to deliver Chronic Care Management to their Medicare patients with two or more chronic diseases lasting for 12 months or longer. SPAC International achieved this certification right after Medicare released the CCM code 99490 back on the Halloween night of 2014.
