Platform educates and helps patients with their care plans, and helps educate clinicians to improve performance and quality

The American Hospital Association (AHA) today announced it has exclusively endorsed the Patient and Clinician Education Platform from iHealthEngines. Following a proprietary due diligence process, AHA Solutions, a division of Health Forum, the strategic business enterprise of the American Hospital Association (AHA), awards the AHA Endorsement to products and services that help member hospitals and health care organizations achieve operational excellence.

As health care shifts from volume- to value-based care, hospitals are seeking to improve individual patient and population health outcomes. One important strategy is to engage health care users through education. Effective education aids patients, families and caregivers in understanding the patients’ conditions and treatments and in making better-informed decisions about their care. In addition, it raises the likelihood that patients will adhere to their care plans ― helping to improve outcomes and patients’ satisfaction with their care. Improved patient education also can reduce readmissions by more than 30 percent.(1)

To empower patients to actively participate in their care pre-op and post-discharge, iHealthEngines’ cloud-based Patient and Clinician Education Platform provides educational content and remote contact pathways. These are personalized to each patient’s conditions, language, medical codes and other characteristics. The platform also helps clinicians in team-based settings deliver better, more personalized care throughout the care continuum — from hospital to home — without added workload. iHealthEngines’ platform applies to organizations ranging in size from large integrated health care systems and medium-sized hospitals, to smaller independent medical practices.

With the help of proprietary automation, the solution enables clinicians to manage the care of patients with serious medical conditions by prescribing personalized information and remote patient contact pathways apps. This prescribed content educates patients on their care and enables them to feed information (such as treatment adherence for congestive heart failure, or post-surgery progress and recovery) back to physicians in real time and between clinic visits.

Patients use their own PCs, tablets or mobile phones to securely access the platform’s content. They can also use the platform to communicate with their clinicians by reporting on their progress, asking questions about their care plan and providing feedback regarding their satisfaction with their care. As patients progress, they complete health literacy quizzes and questionnaires related to their progress and care experience to earn Personal Learning and Engagement credits redeemable for discounts, co-pay coupons or other health care benefits. As these incentive-based credits motivate patients to learn and improve health behavior and outcomes, they allow physicians to measure patient activation and follow up on patients’ progress.

Hospital training managers can also use the platform to train and engage clinicians and workforce by creating effective personalized training programs, tracking completion and measuring the impact of every training module on organizational performance objectives.

“The AHA endorsed iHealthEngines’ solution because of its credits and incentives based education; “learn anytime, anywhere” mobile convenience; and unique patient/provider communication and monitoring capabilities. It also offers unparalleled levels of personalization, interoperability with other systems, and content-source flexibility, all of which contribute to more effective patient education and engagement,” said Tim Steffl, chief operating & development officer, AHA Solutions. “By delivering timely information to the patient that supports effective care transition, and helping train clinicians in best practices, iHealthEngines’ platform contributes to better health outcomes and lower readmissions.”

“The AHA’s exclusive endorsement of the iHealthEngines Patient and Clinician Education technology is a powerful validation of our technology leadership, strong fit for health systems, and the positive response our technology has received from clinicians,” said Harish P.K. Chidambaran, president and CEO of iHealthEngines. “IHE and its interactive personalization and delivery technology is critical for health systems to deliver personalized and value-based care, and implement a transformative patient relationship management program. We are excited about partnering with the AHA and its members to further the mission to improve patient and physician engagement, enhance outcomes and reduce cost of care.”
