Medical & Surgical Clinic of Irving Successfully Implements Chronic Care Management Program
Irving, Texas (PRWEB) March 10, 2016 -- Medical & Surgical Clinic of Irving (M&SCI) is proud to announce that on March 1st, they brought live the Chronic Care Management or “CCM” program in partnership with Allscripts. Effectively managing the care of Medicare patients with chronic conditions takes more than just an occasional face-to-face office visit. Seniors need and deserve outreach, telephone conversations, medication reconciliation and coordination among caregivers, along with other activities. And, that’s just what M&SCI does to ensure the best care for their patients.
CCM allows M&SCI to better track these activities and provides additional resources for their patients. Enrolled patients will now have 24/7 access to care services by telephone. And, the clinicians providing after-hours care will have full access to the patient’s plan of care and medications through the system. In addition to the care patients are already receiving, clinicians conduct a monthly medication reconciliation to audit the medications the patient is taking. This adds an additional layer of safety, ensuring the patients are on the proper doses of the right medications and that drug interactions are minimized.

The program was successfully implemented after months in development. M&SCI enrolled nearly 75 patients the first week and the feedback has been extremely positive. Additional eligible patients are now being contacted regarding their chronic care management. M&SCI expects to enroll more than 2,500 Medicare and Medicare Advantage patients by August 31, 2016 to offer the highest level of care possible to these chronically ill patients.
