Non-Medical Home Care Company Participates in Medicare Bundled Payment for Care Improvement (BPCI)
Phoenix, Arizona (PRWEB) March 14, 2016 -- 24Hr HomeCare, in an effort to better meet the needs of Phoenix area patients undergoing orthopedic surgery, has collaborated with HealthSouth East Valley Rehabilitation Hospital in Mesa to provide a cost-effective solution for patients that are rehabilitating and recovering in their home. The goal of the program is to improve patient outcomes through assistance with activities of daily living. The partnership is intended to prevent health decline which can lead to further utilization of costly healthcare services, such as hospital readmissions.
HealthSouth East Valley is a participant in Medicare’s Bundled Payment for Care Improvement (BPCI) initiative, which was created by the Centers for Medicare and Medicaid Services. Over the course of the BPCI initiative, CMS will work with participating organizations to assess whether various models being tested result in improved patient care and lower costs to Medicare. Acute care hospitals, including acute rehabilitation hospitals such as HealthSouth East Valley , have been challenged with ensuring their discharged patients rehabilitate and recover from their hospitalizations and surgeries when they return home.
The collaboration with HealthSouth East Valley will allow 24Hr HomeCare caregivers to serve as an extension of the HealthSouth East Valley team through a high touch, cost effective approach. 24Hr’s care providers will ensure HealthSouth discharge instructions are properly followed through on via a combination of hands-on care, homemaking, and care coaching that all support the participant’s ability to perform activities of daily living (ADLs). Care visits will be spread out over a period of 60 days as 24Hr collaborates with the patient, the patient’s family, and other care providers such as the primary care physician to ensure a successful recovery. 24Hr HomeCare has other programs throughout California, Arizona, and Texas that have similarly been funded by healthcare organizations as a means of improved patient outcomes and long term cost savings to the health system.
