Care Navigator Inc. and OneCare Vermont Announce Software to Manage Care Coordination in State-wide ACO
Cranbury, NJ (PRWEB) March 23, 2016 -- Care Navigator Inc. and OneCare Vermont, Vermont’s state-wide Accountable Care Organization (ACO), announced today the selection of the Care Navigator™ software platform. Care Navigator™ is the leading shared care management software platform that leverages Cloud, desktop and mobile technologies to provide actionable care coordination.
OneCare Vermont is adopting the software to help care coordinators improve communication and collaboration among community service providers, patients and other stakeholders. It will provide care coordination support among the ACO’s extensive network of participating providers and engage its over 110,000 attributed patients. Care Navigator Inc. is wholly owned by New Jersey-based BluePrint Healthcare IT, which developed the software.
As one of the leading state-wide ACO’s in the country, OneCare Vermont works with Medicare, Vermont Medicaid and the Commercial Exchange Shared Saving Programs and a network of providers including most of Vermont's hospitals, 2 New Hampshire hospitals, hundreds of primary and specialty care physicians and Advance Practice Providers, Federally Qualified Health Centers (FQHC), and several rural health clinics. OneCare Vermont was created by The University of Vermont Medical Center and Dartmouth-Hitchcock Health.
In order to personalize and automate the experience for all users, Care Navigator™ employs algorithms that combine clinical, quality and claims data from Electronic Health Records and population health analytics software as well as from its own native care management data. OneCare Vermont’s deployment will begin with care coordinators, who engage directly with patients and manage care delivery. Initially, it will focus on high risk patients with medically complex conditions. OneCare Vermont plans to expand its use of Care Navigator™ in phases, eventually to include physicians, additional clinical staff, patients and family members.
