MedaCheck’s medication & disease management platform will support the Indiana-based, independent pharmacy, offering its telehealth technology to support thousands of older adult patients

MedaCheck, a leading medication and disease management platform used to improve adherence, transition of care and quality outcomes, has announced its partnership with GrandView Health Services.

With decades of experience, Grandview Health Services is an innovative, independent pharmacy that provides over the counter and prescription drug pharmacy services to thousands of residents. Serving Indiana, Ohio, Michigan, Tennessee and the surrounding region, GrandView Health Services provides comprehensive pharmacy services and consulting to long term care communities, continuing care retirement communities, assisted living communities, group homes, and more.

MedaCheck provides a reminder and management device for patients, delivering real-time alerts and messages so that patients take the right dose, at the right time, every time. Helping patients manage all their ongoing prescription and over the counter medications, MedaCheck is available via standalone tablet or app (iOS and Android). Combining technology with behavior support, MedaCheck’s progressive reminder system includes add-on functionality tying in their clinician, care team and/or caregivers. If a patient does not indicate they have taken medication as prescribed, a follow-up call can be placed to the patient, as well as her clinician, care team, and/or caregiver. With their most recent product release, the platform will now offer additional devices like glucose monitors and blood pressure cuffs to help support chronic conditions like diabetes or CHF.

The partnership between MedaCheck and GrandView Health Services will focus on improving transitional care. With access to MedaCheck, patients will be able to use telehealth technology that can provide them with real-time, actionable information that can help them self-manage their health and stay independent.

“With the inclusion of new quality indicators/quality measures to the CMS 5 Star Rating system for skilled nursing facilities related to successful care transitions and prevention of hospital/SNF readmissions, the need to monitor high risk populations, in the community, has never been greater,” says GrandView Health Services’ Chief Executive Officer and Group President, Mark Prifogle.

We chose to partner with MedaCheck for our comprehensive transitions of care and hospital readmission mitigation program, not only its robust platform and Bluetooth compatibility, but it’s inexpensiveness compared to other similar monitoring systems,” adds Prifogle. “If the home health, hospice or patient refuse to pay for a solution, it has no value because it will not be used.
