MedaCheck Announces Partnership with GrandView Health Services to Improve Transition of Care and Hospital Readmission Mitigation

MedaCheck provides a reminder and management device for patients, delivering real-time alerts and messages so that patients take the right dose, at the right time, every time. Helping patients manage all their ongoing prescription and over the counter medications, MedaCheck is available via standalone tablet or app (iOS and Android). Combining technology with behavior support, MedaCheck’s progressive reminder system includes add-on functionality tying in their clinician, care team and/or caregivers. If a patient does not indicate they have taken medication as prescribed, a follow-up call can be placed to the patient, as well as her clinician, care team, and/or caregiver. With their most recent product release, the platform will now offer additional devices like glucose monitors and blood pressure cuffs to help support chronic conditions like diabetes or CHF.
The partnership between MedaCheck and GrandView Health Services will focus on improving transitional care. With access to MedaCheck, patients will be able to use telehealth technology that can provide them with real-time, actionable information that can help them self-manage their health and stay independent.
