The presentation will discuss the concept of coordinated care and how important it is in the future of healthcare, specifically in post-acute facilities. Care coordination is not imaginative; the Centers for Medicare and medicaid Services (CMS) develops programs to help build these relationships that will help make care coordination easier in value-based programs.
