New home health services will be added to myPotential at Home—A National Lutheran Service starting in September 2016. myPotential at Home will provide both home care and home health services to seniors in the comfort of their own homes in Winchester City, Clarke, Warren and Frederick counties.

Backed by National Lutheran Communities & Services’ 125 years of experience, myPotential at Home offers a non-franchise option for home care and home health that works to preserve seniors’ independence while helping them reach their potential at any age and any stage. The home health services that myPotential at Home will offer in September include skilled nursing, physical therapy, occupational therapy, speech therapy and social services.

“Unfortunately many seniors spend too much time dealing with chronic conditions or issues that keep them in the hospital longer than necessary,” myPotential at Home’s Executive Director Denise Kozlowski said. “With the addition of in-home health care services, we can deliver those added services in the comfort of one’s home, while lessening unnecessary hospital stays and simultaneously coordinating all care efforts.”

Currently myPotential at Home offers home care services, which feature both caregiver and wellness services such as meal preparation, respite care, escorting individuals to appointments, assistance with activities of daily living, medication monitoring, wound care, skin care and more.
