Vital Data Technology Announces Contract with Community Care Collaborative for Quality Management Platform
NEWPORT BEACH, Calif. (PRWEB) August 09, 2016 -- Vital Data Technology (Vital Data), is pleased to announce that it has entered into a contract with Community Care Collaborative (CCC) of Austin, Texas to deploy the integrated Case Management system of Vital Data’s Quality Management Platform. The CCC is a public/private partnership between Central Health (the taxing healthcare district in Travis County, Texas) and Seton Healthcare Family. CCC was designed to develop an integrated delivery system to better manage the care of low income and uninsured populations by providing high quality, cost-effective, person-centered care that improves health outcomes. The CCC will utilize the Vital Data’s Case Management and Care Coordination module to improve care management across its diverse population.
“Vital Data’s comprehensive tool set will provide automated workflows, letter generation and reporting that will help the CCC improve our case management processes, communication and workflows and allow CCC Case Managers to become more efficient and to reach more clients in need,” said Dr. Mark Hernandez, the CCC’s Chief Medical Officer.
