LIFE St. Francis Relocates to New Facility in Bordentown
TRENTON, NJ (PRWEB) September 02, 2016 -- LIFE St. Francis, a Program of All Inclusive Care for the Elderly, recently relocated to a brand new center in Bordentown, NJ. Located on the Team 85 Campus, LIFE St. Francis now occupies a 30,000 square-foot, two-story facility where its team will provide care to seniors throughout Mercer County and expanded portions of Burlington County. According to Vincent Costantino, Chief Administrative Officer, St. Francis, the $7.2M investment follows St. Francis’ commitment to population health as well as its objective to reduce the cost of care in the communities it serves.
LIFE St. Francis serves frail seniors, enabling them to receive comprehensive health care while remaining in the comfort of their home and community. At the LIFE Center, seniors receive health care, nutritious meals, and participate in activities with others so they may remain active and social. The LIFE Center provides one location where doctors, nurses and other healthcare professionals, including physical therapists, occupational therapists and social workers, can provide treatment and services while monitoring changes in an individual’s health.
Jill Viggiano, MS, President of LIFE St. Francis stated, “LIFE St. Francis has improved the lives of more than 600 seniors since opening in 2009. With this new, state-of-the-art and much larger PACE Center as our ‘hub,’ LIFE’s compassionate, highly trained staff will serve our current senior members while also sharing the benefits of enrollment in LIFE St. Francis with new members.”
In order to participate in LIFE, enrollees must be at least age 55 or older; certified by the state to need nursing home care; able to live safely in the community at the time of enrollment; and live in Mercer County or portions of Burlington County. LIFE now serves the following areas in Burlington: Bordentown, Chesterfield/Crosswicks, Florence, Roebling, Browns Mills, Burlington, Columbus, Mt. Holly and Pemberton.

LIFE St. Francis is a Program of All-Inclusive Care for the Elderly (PACE) and provides comprehensive care to residents of Mercer County and portions of Burlington County who are 55 and older and wish to remain living safely and independently in the community but need coordinated services. Services are provided at the LIFE Center located at 7500 Kevin Johnson Boulevard in Bordentown, and in participant’s homes. LIFE St. Francis is able to provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their homes for as long as possible.
