The Central Virginia Coalition of Healthcare Providers (CVCHiP), an Accountable Care Organization providing services in Virginia and North Carolina, has selected Smartlink Mobile Systems as their Chronic Care Management (CCM) solution partner.

With 93% of Medicare dollars spent on beneficiaries who have multiple chronic illnesses, providing more proactive care management services to this population has become critical to success under payment reform. Smartlink will provide CVCHiP with their comprehensive Value-Based CCM solution that will help the ACO’s providers deliver consistent, high quality care, improve quality measures, identify opportunities for early clinical intervention to prevent health deterioration, and reduce utilization of healthcare services by keeping beneficiaries healthier. The solution is uniquely designed to help clinicians leverage CCM to improve utilization and quality performance measures under the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA).

MACRA is a bipartisan legislation that changes how Medicare pays those who provide care to Medicare beneficiaries. It is intended to accelerate the shift from fee-for-service to fee-for-value by rewarding clinicians for providing better care at a lower cost. The Medicare CCM program is intended to help providers make the transition to value-based care by paying providers to provide monthly, non-face-to-face services to beneficiaries with two or more chronic illnesses. In addition to incentivizing better care management, the CCM program helps to subsidize the necessary investments to succeed under MACRA.

“All of our practices are small practices that love providing care on a more personal level,” said Lerla G. Joseph, MD, CEO and Chair, CVCHiP. “Our goal as an ACO is to bring to the table the expertise to help these small practices continue to do what they love, and at the same time succeed financially under MACRA. Smartlink’s Value-Based CCM is a critical part of our strategy to achieve that goal.”

“MACRA represents a number of challenges, but for organizations such as CVCHiP that are doing all the right things, it also represents tremendous opportunity,” said Siu Tong, CEO, Smartlink. “Maintaining the vitality of independent practices is a goal that we share, and we are very excited to work with the CVCHiP team towards that end.”
