Root Cause Analysis (RCA) is a problem-solving mechanism that is used to identify root causes for different kinds of problems. Any factor which when removed prevents an undesirable event from reoccurring can be termed as the root cause. It must be noted here that there is a difference between root cause factor and causal factor. While the former affects the event's outcome as a root cause, the latter affects the event's outcome, but cannot be termed as the root cause. This is why removing a causal factor may benefit one outcome for the time, but it can never prevent its occurrence with total certainty.

Root cause analysis is used by organizations and businesses for responding to critical incidents as well as repetitive problems. It is primarily meant to identify root causes, which when dealt with, can prevent the reoccurrence of the issue in the future. However, many people are often not able to achieve expected results despite that. This usually happens because they tend to use 'System 1 thinking' instead of making use of a more robust analytical apparatus.
