Hixny Launches Mobile App for More Efficient Real Time Care Coordination

Hixny’s new Integrated Care Manager, or ICM, allows healthcare professionals to monitor the care of their patients from remote locations in real time. ICM also allows healthcare professionals to customize real time alerts from their mobile device.
Hixny has been providing access to electronic health records to 100% of hospitals and more than 70% of physicians in the region for years. It has also been providing real-time alerts which inform healthcare professionals when a patient has been admitted or discharged from a hospital or emergency department (ED). ICM is a first-of-its-kind Health Information Exchange (HIE) based solution which empowers physicians to customize those alerts so they can be monitored in real time from their smart devices.
This means that while many physicians monitor hundreds of patients through Hixny alerts, they can now utilize ICM to monitor their most high-risk patients. It also allows them to track the progress of their patients in hospital and ED settings and receive real-time reports and updates.
ICM is a secure, HIPAA-compliant application that equips users to set preferences and access and share patient information securely, from any device. As a result, users can more efficiently and effectively coordinate patient care, even forming a “virtual healthcare team” to provide the care patients need when they need it.
