The Primary Care Coalition (PCC) President and CEO, Leslie Graham, announced today that two seasoned executives will join the organization’s leadership team.

Avi Fichman will join the PCC as Chief Operating Officer. Mr. Fichman brings more than 25 years of experience to the PCC and has held executive roles for a number of well-known organizations including: Make-A-Wish of the Mid Atlantic; UNHCR; and The Center for Public Integrity. As PCC’s Chief Operating Officer, he will oversee policies and procedures, and provide financial analysis, budgeting, and contracting expertise.

Jeff Goldman will become the PCC’s first Vice President for Population Health and Director of the Nexus Montgomery Regional Partnership. He will be responsible for shaping the vision and leading the implementation effort for Nexus Montgomery—a new collaborative effort among the six hospitals operating in Montgomery County, PCC and other community partners. The goal of Nexus Montgomery is to reduce unnecessary hospital costs and improve the health of our community through jointly operated programs that focus on four populations: the medically frail, Medicare seniors age 65 and above, individuals with severe mental illness, and the remaining uninsured in need of specialty care.

Jeff brings a wealth of experience and has previously held senior roles at Kaiser Permanente, the American Hospital Association, and Valence Health, Inc. He also has strong community ties; he is a Leadership Montgomery graduate and former member of its Board of Trustees. In his role at Kaiser Permanente, he served on the Healthy Montgomery Steering Committee.

“This is an exciting time for the PCC and the community we serve; a time of growth and opportunity. We are excited to have Avi and Jeff join our team as we continue to shape our programs and expand our activities to address the convergence of population health, health care utilization and individual activation for improved health and experience of health care” said Ms. Graham.

The PCC works on many levels to increase access to health care and improve the health and lives of low-income, culturally diverse residents of Montgomery County, Maryland and the surrounding communities. As members of the executive team, Mr. Fichman and Mr. Goldman will work with diverse stakeholders to develop systems of care that meet the needs of vulnerable community members and respond to the challenges of today’s rapidly changing health care industry.
