Casenet Integrated Care Management Platform Goes Live at South Country Health Alliance to Improve Outcomes, Efficiency, Productivity and Interdepartmental Alignment
(PRWEB) January 12, 2016 -- Casenet®, LLC, a leading provider of extensible care management solutions, today announced that South Country Health Alliance (SCHA) has successfully completed an on-time, on-budget implementation of the TruCare® care management platform, which will better manage the complex care of Medicaid populations in Minnesota. Starting in October, SCHA’s Health Services Department began using the TruCare platform to manage and facilitate all case, disease and utilization management services administered to 38,000 members across 12 Minnesota counties.
SCHA is a county-owned health plan committed to serving some of the nation’s most vulnerable Medicaid populations, including those of low income, disability and mental illness. SCHA selected TruCare to enable improved management of the complex care of their diverse populations because of TruCare’s ability to integrate medical, behavioral and ancillary services including those of home and community-based services. Also, the vast configurability of TruCare allows SCHA users to tailor the system by segmented populations and lines-of-business, and the robust reporting enables the development of internally-created custom reports to easily view and share valuable care management data with other users and systems.

The TruCare implementation was a collaborative effort between SCHA’s Health Services and Information Technology Departments and the Casenet team. Together, the SCHA team with assistance from a responsive Casenet team combined the activities of previously isolated departments by configuring the TruCare system and workflows to meet their needs and leveraging TruCare’s integration web services to feed valuable member data, as well as tasks and referrals into TruCare from other SCHA data sources.
SCHA’s Business Analyst, Megan LaCanne, described the implementation structured by Casenet as an “unprecedented partnership across the entire organization that engaged stakeholders in timely and organized teamwork”.
“Casenet facilitated a great amount of communication between SCHA and our different departments, ensuring that we were not operating in siloes,” said LaCanne. “Casenet provided a detailed implementation plan developed around best practices. Our weekly meetings involved not just project sponsors, but coordinated teams of programmers and resources across the business working together with Casenet to determine how TruCare could best be tailored to fit South Country.”
SCHA leaders are pleased that the TruCare implementation remained on-time and on-budget, without harried milestones or compromising the quality of the system configuration and overall user acceptance.
“The Casenet project team structured the implementation with a cadence that allowed the process to be executed carefully and gave us sufficient time to reach each milestone,” said LaCanne. “The Casenet team’s thoughtful planning left room for appropriate testing, which took the stress out of the initial go-live. With TruCare, we had a chance to fully test and work through any issues with a responsive implementation team. This well-managed implementation allowed us to remain fixed to our timeline and confident about moving forward toward our go-live.”
Implementing TruCare is a major step towards SCHA’s goal of delivering real-time, accurate data to system users, making care teams more effective with access to all complex case information in one location. In addition to member health impacts, SCHA anticipates a significant increase in staff productivity and efficiency as manual processes are eliminated and three separate systems are replaced by the TruCare platform. By integrating all program data in one system, SCHA care teams now have a holistic view of each member and associated programs, including the behavioral and disease management elements that were previously missing.
Currently, SCHA is exercising a large breadth of TruCare functionality, using not only the basic TruCare client and administration functionality, but also taking advantage of the integration and interfaces that can provide real-time data exchange. SCHA has leveraged core data feeds to access and load information about members, products, networks and providers into TruCare from their data warehouse, as well as additional referral and task web services. Looking ahead, SCHA plans to use TruCare even more broadly across the organization, introducing TruCare functionality across member services and operations departments to further the impact of the integrated care management system.
