A Chronic Special Needs Plan Can Provide More Medicare Coverage to Members Dealing with Specific Chronic Conditions in California

A Chronic Special Needs Plan or C-SNP restricts enrollment to individuals with specific chronic conditions, such as cardiovascular disorders, and diabetes.
