Alliance of Private Sector Leaders Spearheading Adoption of Value-Based Payment Models

Value-based care led by primary care physicians is an effective and scalable way to improve both patient health and satisfaction for seniors.

The Health Care Transformation Task Force, a consortium of leading health care payers, providers, purchasers and patient organizations working to accelerate the pace of U.S. health care delivery system transformation, announced today that ChenMed has joined its membership.

For seniors most in need of care, high quality health care is too often beyond reach. ChenMed is a health care provider that was founded to bring concierge-style medicine — and better health outcomes — to the neediest populations. ChenMed serves seniors with low-to-moderate incomes, most managing multiple chronic conditions, in nine U.S. markets through its 39 senior medical centers. As a fully capitated Medicare Advantage provider, ChenMed provides a concierge level of care with minimal to no expense to the beneficiary.

As part of the Health Care Transformation Task Force, members commit to putting 75 percent of their business into value-based arrangements that focus on providing person-centered care that is high quality and lower cost through innovative delivery models.

With more than 40 organizations representing patients, payers, providers and purchasers, the Task Force aims to align private and public sector efforts to transform the U.S. health care system. By developing and disseminating strategy, operational and policy recommendations, it seeks to spark rapid, measurable change across the health care landscape.

ChenMed’s unique model and culture of accountability allow it to better manage chronic disease and keep patients healthy, resulting in fewer days in the hospital and lower costs. ChenMed patients have access to services like medications provided on-site, dedicated specialists, on-site tests and screenings, and available courtesy transportation. ChenMed’s doctors annually serve one-fifth as many patients as the national average of 2,300 patients per doctor. This allows them to provide significantly more face-to-face time with patients. Industry-leading access is one reason why Medical Economics reported that ChenMed “may be the best primary care system in the U.S.”
