Innovative telehealth company, Health Recovery Solutions, announces launch of monitoring service, HRS CareConnect, helping providers extend care coverage

Health Recovery Solutions (HRS) today announces the launch of a new clinical monitoring service to augment its telehealth solution. HRS CareConnect provides various levels of support, monitoring, and patient engagement. The solution gives providers the unique opportunity to deliver continuous care by supporting clinician coverage gaps to help minimize the risk of avoidable hospital readmissions and ED visits for high-risk chronically ill patients.

To deliver this program, HRS has partnered with Advanced Telehealth Solutions, a URAC-accredited health call center providing a broad range of clinical support services since 2002. With this partnership, the HRS CareConnect service will be available across the US to provide care interventions based on each provider’s unique clinical protocol. Another unique feature of the program is that the Advanced Telehealth Solutions teams are led by chronic care nursing experts who are crossed-trained in motivational coaching. This experience allows the HRS CareConnect teams to work across a broad spectrum of patient conditions and comorbidities.
