STRAND® Platform Achieves Pharmacist eCare Plan Level 3 Active Medications Capability
EVANS, Ga. (PRWEB) November 30, 2017 -- Creative Pharmacist, the innovative market leader in developing and supporting clinical services offered by community pharmacies, announced today that its STRAND® Platform has successfully achieved Level 3 Active Medications capability for the development and implementation of the Pharmacist eCare Plan initiative. This project is supported by the Pharmacy Health Information Technology Collaborative that is being tested by Community Care of North Carolina to support in the integration of community pharmacy patient encounters with care management and primary care delivery and information systems.
The Pharmacist eCare Plan is a collective document that details a patient’s current medication regimen and health concerns, including drug therapy problems, therapeutic goals and medication support needs, in addition to the pharmacy's interventions and the patient's health outcomes. With the new capability, pharmacists utilizing the STRAND platform are now able to identify the active medications contained in a patient’s medication regimen, thereby answering a key element in care coordination. After the pharmacist develops the eCare plan, the STRAND platform then communicates the eCare plan—in both computer and human readable versions—via direct secure messaging to other members of the healthcare team.
