CHICAGO (PRWEB) January 18, 2018
Oak Street Health, a network of health centers providing a new model of value-based primary care for Medicare patients, has named David Buchanan M.D., M.S. as Executive Medical Director for Illinois. Dr. Buchanan will be responsible for leading the primary care, APN and complex care teams in Illinois. Additionally, he will advance critical clinical initiatives that support safety, population health, and quality operations across the organization.
Dr. Buchanan is an accomplished clinician and leader, with an impressive track record of success for scaling organizations and championing strategic initiatives. Most recently he served as Chief Clinical Officer of Erie Family Health Center. In that role, he more than doubled the size of the practice, recruited and led more than 200 medical, behavioral health, and oral health providers, and designed an innovative care model including care management and the population health program. He chaired multiple boards and Clinical Committees, including for a Coordinated Care Entity, an Accountable Care Organization and a Health Center Controlled Network focused on Health IT.
Prior to his time at Erie, Dr. Buchanan held multiple positions at the Cook County Health and Hospital System, including Head of the Section of Social Medicine and Director of Medical Student Programs at Cook County Hospital.
“We are thrilled to have Dr. Buchanan on the Oak Street team,” said Mike Pykosz, CEO of Oak Street Health. "His track record of successfully building and leading strong healthcare organizations and his passion for patient-centered care and innovation will advance Oak Street’s patient care goals.”
Dr. Buchanan’s major initiatives will include rapidly expanding Oak Street Health’s primary care team in both new and existing sites across Illinois and driving operational excellence to help the organization scale. Additionally, Dr. Buchanan will champion the use of new bedside technology to improve patient outcomes and drive the innovation of its complex care team model, which brings care into the homes of high-need patients.
“I have seen first-hand the lasting impact Oak Street Health has on communities and I am honored to have the opportunity to drive that mission forward,” said Buchanan. “As Oak Street Health continues to expand, we will increase patient access to top quality care, creating healthier, happier communities as a result.”

About Oak Street Health
Oak Street Health is a rapidly growing company of primary care centers for adults on Medicare in medically-underserved communities where there is little to no quality healthcare. Oak Street’s care is based on an entirely new model that is based on value for its patients, not on volume of services. The company is accountable for its patients’ health, spending more than twice as long with its patients and taking on the risks and costs of their care. Since 2013, Oak Street Health has been investing in communities across the Midwest – providing much-needed primary care for tens of thousands of people in 24 centers. The company recently announced an expansion to Philadelphia in 2018. For more information, visit http://www.oakstreethealth.com.
