Healthify, the leading software solution for addressing individuals' social determinants of health, announced today that its Co-Founder and CEO, Manik Bhat, will participate in a panel discussion, “Improving Care Transitions Between Settings to Reduce Avoidable Readmissions and Improve Outcomes,” at the Medicaid Innovations Forum on February 2 from 11-11:40 a.m.
Avoidable hospital readmissions for Medicaid recipients have become a costly expenditure for hospitals nationwide. The United States spends $2 trillion on hospitalizations alone, while the average cost of readmission for a Medicaid patient is estimated to be around $12,300. The Centers for Medicare and Medicaid Services have implemented programs to assist hospitals in reducing readmissions but still, more than half of hospitals in the country faced readmission fines and substantial reductions in payment (as high as 3%) in 2017. The panel will explore how bettering care transitions between care settings is critical to reducing avoidable readmission rates and improving overall health outcomes for patients.
Manik will offer insights on how hospitals use Healthify’s solutions to assess a patient's’ needs, coordinate them with the appropriate home health and community care resources and track their transition from clinical to home care. "Evidence shows that the rate of avoidable readmissions can be reduced by improving transitions from the hospital to local social services,” said Manik.
