Oak Street Health Adds Two Executive Roles to Indiana Division to Enhance Patient Care
CHICAGO (PRWEB) February 06, 2018 -- Oak Street Health, a network of health centers providing a new model of value-based primary care for Medicare patients, has bolstered its leadership team in its Indiana division to advance patient care and growth objectives. Brian Clem, now President of Oak Street Health Indiana and Mark Bustamante M.D., now Executive Medical Director of Oak Street Health Indiana, will lead the expansion of coordinated partnerships with Indiana health systems, insurance companies and state government for improved access to managed care.

A homegrown Hoosier, Brian Clem has served as Regional Vice President of Oak Street Health since 2015. In this role he lead the development of Oak Street Health in Indiana and spearheaded strategic initiatives that have reduced hospitalizations of Oak Street Health patients by 40 percent compared to benchmarks. Previously, he served as Director of Medicare Markets for IU Health Plans, where he was responsible for the growth and performance of IU Health Plans Medicare Advantage business.
Dr. Bustamante joined Oak Street Health as Regional Medical Director in 2016 and has made a tremendous impact on patient well-being during his time. His deep expertise, vision and entrepreneurial spirit have resulted in innovative solutions for improving care for medically underserved patients. Dr. Bustamante is highly involved in the Indiana community. He is a Fellow of the American College of Physicians, a member of the American Academy of Pain Medicine, and serves on the Indiana Geriatric Society Board of Directors.
