Creative Pharmacist Announces End-to-End Solution for the Diabetes Prevention Program
EVANS, Ga. (PRWEB) February 09, 2018 -- Creative Pharmacist, the innovative market leader in developing and supporting clinical services offered by community pharmacies, today announced an end-to-end solution for pharmacists to engage in the Center for Disease Control’s Diabetes Prevention Program or DPP. The new offering empowers community pharmacists with personal support through the CDC’s recognition process and provides turnkey documentation, billing, and reporting tools within the pharmacist’s workflow.
The number of adults in the United States who have pre-diabetes is shocking. One in three Americans is at risk of developing the disease. If pre-diabetes is left untreated, it often leads to type 2 diabetes within five years. According to the CDC, approximately 84 million American adults are considered to be living with pre-diabetes, while another 30.3 million Americans have type 2 diabetes.
“In every area of our country, pharmacists are able to fight this epidemic by standing in the gap alongside their patients,” said David Pope, PharmD, CDE, Chief of Innovation and co-founder of Creative Pharmacist. By empowering the clinical community pharmacist –who is often one of the only healthcare providers found in rural communities– with the right tools to combat both pre-diabetes and diabetes, we’re confident we’ll be able to slow the progression of this ever-increasing condition.”
Using STRAND, pharmacists can perform the interventions that hallmark the DPP program –including education, weight monitoring, and clinical documentation– all within the pharmacist’s typical workflow. Additionally, STRAND’s communication abilities allow pharmacists to coordinate efforts with other healthcare team members through the use of direct secure messaging and faxing capabilities within the platform. Pharmacists can also utilize the STRAND patient-facing app, available on both Apple and Android devices, to connect with patients, allowing users to communicate their weight, blood pressure and exercise activity directly to their pharmacist health coach.
The addition of the DPP program to the clinical services adds to a growing list of clinical services supported through STRAND. For example, more pharmacies used STRAND services to complete the diabetes self-management education accreditation process than through any other pathway in 2017.
“This is a great program and a way for the pharmacy to make an impact on a patient’s overall health,” said Lori Anglin, Pharmacy Director at Barnes Healthcare in Valdosta, GA. “The tools the STRAND platform offers have made it very simple for both the pharmacy and the patient to stay on track and engaged in the patient’s overall health goals.”
Pharmacists can submit clinical service claims, such as the DPP, to payers, including Medicare, directly within STRAND. Through the STRAND ProClaim pathway, pharmacists can receive personalized support through the credentialing and contracting phase of connecting with payers as well as in submitting claims.
