The new offering empowers community pharmacists with personal support through the CDC’s recognition process and provides turnkey documentation, billing, and reporting tools within the pharmacist’s workflow.
The number of adults in the United States who have pre-diabetes is shocking. One in three Americans is at risk of developing the disease. If pre-diabetes is left untreated, it often leads to type 2 diabetes within five years. According to the CDC, approximately 84 million American adults are considered to be living with pre-diabetes, while another 30.3 million Americans have type 2 diabetes.

Using STRAND, pharmacists can perform the interventions that hallmark the DPP program –including education, weight monitoring, and clinical documentation– all within the pharmacist’s typical workflow. Additionally, STRAND’s communication abilities allow pharmacists to coordinate efforts with other healthcare team members through the use of direct secure messaging and faxing capabilities within the platform. Pharmacists can also utilize the STRAND patient-facing app, available on both Apple and Android devices, to connect with patients, allowing users to communicate their weight, blood pressure and exercise activity directly to their pharmacist health coach.
The addition of the DPP program to the clinical services adds to a growing list of clinical services supported through STRAND. For example, more pharmacies used STRAND services to complete the diabetes self-management education accreditation process than through any other pathway in 2017.
