Over the last 20 years, health expenditure growth has more than doubled and continues to rise rapidly, while rates of chronic diseases continue to increase. One of the most significant challenges in healthcare is determining how to cost-effectively use data to improve health outcomes. Tracking the social determinants of health data can help health plans understand trends in population health and identify the types of resources that are needed to appropriately address unmet social needs. The workshop will explore why healthcare systems should collect and assess social needs and how they can leverage that data to efficiently improve outcomes in patient populations.
Manik will offer insights on how health plans can use Healthify’s solutions to identify needs and track all referral activity and trends in population health.
