Expansion Delivers Personalized Diabetes Coaching to High-Risk Populations in South Florida.

Fit4D, a diabetes management company, announced a program expansion today with Humana Inc. (NYSE: HUM), a leading health and well-being company, to bring Fit4D’s personalized coaching to high-risk Humana members in Broward County, Florida.

Fit4D established a partnership with Humana in April 2017 to provide its diabetes coaching solution to members in North and Central Florida. This new expansion is part of Humana’s Bold Goal initiative to make the Broward community 20% healthier by 2020, and will now serve more diabetes members in Florida.

There are over 30 million people with diabetes in the United States, with a disproportionate amount affecting high-risk populations, including seniors and low-income households. Fit4D successfully combines two important elements: One-on-one support from Certified Diabetes Educators (CDEs), clinicians who specialize in motivating and teaching people with diabetes how to manage their condition, along with technology to scale the reach of CDEs so that greater numbers of patients can have convenient access to specialized education and care. With the increasing social and economic disparities in healthcare, this is particularly important, as Fit4D reaches underserved populations who would not ordinarily be able to access personalized care from a diabetes clinical expert.

Humana’s Bold Goal is to make the communities it serves 20 percent healthier by 2020. Humana uses the U.S. Centers for Disease Control and Prevention (CDC) ‘Healthy Days’ survey tool to measure an individual’s mental and physical ‘Unhealthy Days’ in a 30-day period. From 2015 to 2016 Humana members in Bold Goal communities experienced, on average, a 3 percent decrease in the number of Unhealthy Days. Results are made possible by community-based partnerships, clinical interventions and local strategies developed and executed from Clinical Town Halls and Local Health Advisory Boards.
