Essex County Home Health Unit Adopts Telehealth

Essex County Health Department Partners with Telehealth Company, Health Recovery Solutions, to improve patient outcomes and reduce hospital readmissions

The Essex County Health Department Home Health Unit, which cares for a diverse patient population, adopted innovative telehealth technology from Health Recovery Solutions (HRS) in September 2017.

The Essex County Health Department serves patients of all ages, from infants to the elderly. HRS was chosen because of the ability of the software to monitor patients from home. The telehealth software is used on 4G tablets that are connected to Bluetooth devices such as a weight scale, blood pressure machine, stethoscope, and more. Patients can track their vitals daily and also answer questions about their disease condition. Home Health staff and other health care professionals can then use the software to monitor the patient, track whether they are taking medications, and determine if a patient is at risk for a hospital readmission or an Emergency Department visit.

Through the software, clinicians can talk with patients through text, phone, or video. Patients can do the same, reaching out to their care staff when they need support. This is particularly important for patients in Essex County who reside in rural areas, far from services - or for those who have difficulty traveling to medical appointments.

The software is also able to take images, helping clinicians provide remote wound care. It also allows for medication tracking, biometric monitoring, and more—patients needing a wide range of services such as weight management or glucose control will benefit from telehealth.
