MCG Health, part of the Hearst Health network and a leading provider of informed care strategies, has joined longtime partner Casenet®, a leading provider of population health and care management solutions, to deliver a new solution, the TruCare Assessment and Care Plan Interface™ (TruCare ACPI), Informed by MCG. The TruCare ACPI now integrates with MCG’s evidence-based Chronic Care Guidelines as part of Casenet’s population health management platform. This integration is designed to help organizations close care gaps and reduce readmissions through effective coordination of care for individuals with chronic diseases or complex care issues.

To ensure regulatory compliance and successful management of patient populations, care teams need to rapidly build, document, and share evidence-based assessments and care plans. MCG Chronic Care guidelines help care teams evaluate patient needs, identify goals, develop patient-specific care plans, and support self-care for individuals with complex chronic conditions. Clinical decision support is provided across the care continuum—from short-term inpatient settings to outpatient, home, and community-based care settings. MCG helps clinicians promote patient engagement and self-care with member-facing educational handouts (available in English and Spanish).

Casenet’s TruCare® solution supports multi-disciplinary approaches to the management of members, providing 360-degree views of member information, workflow automation, and simplified data aggregation. The TruCare ACPI, Informed by MCG, seamlessly integrates MCG Chronic Care content into the care management workflow for a streamlined user experience. Within the TruCare ACPI, care managers can complete a member assessment and view MCG care plan recommendations—problems, goals, and interventions (with links to relevant patient handouts and additional MCG Chronic Care Guidelines). They can select elements to add to the care plan to meet member and health needs. The simplified creation of individualized plans of care increases staff efficiency and ensures evidence-based care coordination.
