Get Real Health’s InstantPHR™ Powers Moorings Park Retirement Community Health Management Portal
Rockville, Maryland (PRWEB) September 18, 2013 -- Thanks to a collaboration between Get Real Health and Estero, Florida-based Softrim Corporation, residents of the Moorings Park retirement community in Naples, Florida, are now able to easily integrate health management into their active lifestyles. The Moorings Park initiative marks the first time InstantPHR™ has been used directly in a residential retirement community setting.

The collaboration addressed two important areas: chronic disease and health management, plus patient engagement and meaningful use. By providing the community with an InstantPHR-powered patient portal application, Moorings Park residents now have a system that informs and reminds them of actions they need to take; tracks key metrics related to their health—and prompts them with alerts if they are not keeping those metrics within a target range prescribed by their care manager.
In addition, the system provides residents’ care managers with tools to monitor patient health metrics and care plan compliance as well as communicate with patients. The platform facilitates patient engagement and meaningful use by providing a certified solution that allows patients to view, download, and transmit their medical information, enables secure messaging for patients, and delivers educational resources to patients.
This model of providing a patient health manager portal will be expanded in collaboration between Get Real Health and Softrim to provide similar services to patients of a new geriatric clinic at Moorings Park and also for residents of other retirement communities associated with Moorings Park.

From a technical standpoint, the Moorings Park portal uses an integrated single sign-on solution between InstantPHR and the Aprima electronic health record (EHR) system—a great example of InstantPHR’s capability to be a unified, agnostic, patient portal solution for any EHRs in the market.
While this collaboration was a first for Get Real Health in a retirement community setting, the Company has focused on senior health issues before. In 2011, Get Real Health worked with the City of Los Angeles Department of Aging, in collaboration with the Partners in Care Foundation and the St. Barnabas Senior Center, to develop a creative way to enhance the physical, mental, and emotional health of multi-ethnic, low-income seniors living in Los Angeles through the Senior Health and Wellness Connection project.
