National Transitions of Care Coalition Expands Knowledge and Resource Center Tools to Improve Care Transitions
(PRWEB) May 11, 2015 -- The National Transitions of Care Coalition (NTOCC) today adds an additional resource to the Knowledge and Resource Center, an online portal which provides access to tools and resources that deliver care transitions solutions to providers, patients and consumers. The Joint Commission Transitions of Care (ToC) portal is the latest addition, a valuable source of information from The Joint Commission enterprise and many other healthcare organizations related to the topic of transitions of care.
“We are pleased to include The Joint Commission’s portal in our Knowledge and Resource Center, as their TOC resources expand upon the transition tools we can offer to our community,” remarked NTOCC Executive Director Cheri Lattimer, RN, BSN.
Knowledge and Resource Center tools address one or more of the NTOCC Seven Essential Principles by which providers and organizations must adhere to improve transitions of care, and also address the NTOCC Seven Critical Interventions, key elements found in evidence-based care interventions. These resources address a specific transition of care issue; assist patients, caregivers, healthcare providers and others with respect to transitions of care; and positively impact patient outcomes.
The Knowledge and Resource Center is populated with resources accepted through the NTOCC Essential Resources Program, which recognizes and acknowledges industry tools that provide support and value to transitions of care. Knowledge and Resource Center information and links to accepted tools are available to healthcare providers and consumers at no cost.
