Social determinants of health take center stage in Santa Cruz County, where health and social care providers will use ACT.md's cloud-based platform to integrate services and exchange data
SANTA CRUZ, Calif., Nov. 21, 2019 /PRNewswire-PRWeb/ -- ACT.md, a nationally-recognized provider of integrated health and social care solutions, today announced a partnership with the Santa Cruz Health Information Organization (SCHIO). SCHIO is one of the oldest and most advanced multi-stakeholder health information exchanges in the country, connecting more than 100 organizations across Santa Cruz County, including physician groups, hospitals, federally qualified health centers, county clinics, providers of mental health and substance use disorder services, and community service providers.
SCHIO is offering the care management and community coordination capabilities of ACT.md's CareHub™ platform to its broad base of stakeholder organizations and programs, to ensure seamless communication of patient information and coordination of tasks across the care continuum of multi-disciplinary partners. Participating partners in the CareHub™ include:
- County of Santa Cruz Whole Person Care – Cruz to Health (WPC-C2H) Program
- Santa Cruz Community Health Center's Complex Care Management Program
- Salud Para La Gente's Moving Health Care Upstream (MHCU) Program and Intensive Case Management Program
