CareCycle Navigator provides context to raw health data, taking the guesswork out of who is at risk right now and how to intervene – that’s critical when you are managing thousands of lives every day.

The 2015 South by Southwest (SXSW) Interactive Festival names CareCycle Navigator® as a finalist for the 18th Annual SXSW Innovation Awards in the Health Med & BioTech Category.
CareCycle Navigator predicts which chronically ill patients in a population are at the greatest risk for impending health problems and gives remote clinicians a structured way to intervene and prevent costly health events. The software, developed by Dallas-based CareCycle Management, makes “connected patient” management a scalable reality and functions as an early warning system for chronic illness exacerbations.
Daily health biometrics, trended data, medical history, social factors, on-going health assessments and current symptom data run through patent-pending algorithms in CareCycle Navigator and a patient-specific dynamic risk score is calculated every time new information comes into the system. Because CareCycle Navigator defines in real-time precisely which patient is most at risk at any moment, clinicians can operate at the top of their license and manage large populations. The application also prompts and tracks disease education and new clinical assessment data such as fall risk, sleep, anxiety and depression.
“If you have 2,000 or 200,000 patients in your population, CareCycle Navigator will highlight and define escalating risk factors in real-time and triage the data so that the highest risk individual is always at the top of the list,” said Georgia Brown, CareCycle Management Chief Operating Officer. “CareCycle Navigator provides context to raw health data, taking the guesswork out of who is at risk right now and how to intervene – that’s critical when you are managing thousands of lives every day.”
CareCycle Management uses CareCycle Navigator to remotely manage the care of thousands of chronically ill patients every day and prevent unnecessary hospitalizations. “Over 80 percent of costs in healthcare are spent on the care of the chronically ill and that’s a problem,” said Matt Johnson, CareCycle Management Chief Executive Officer. “The real challenge is finding the right patients and intervening before their chronic illness becomes acute – this is ultimately what will drive waste out of healthcare costs, keep people at home and improve lives.”
“Our validated hospitalization rates on over 31,000 patients have been between 6.5-8.5 percent for
24 consecutive quarters. It’s an honor to be recognized for our innovation solving a problem everyone is working on.”
CareCycle Navigator was selected out of hundreds of applications by a panel of SXSW judges comprised of industry experts and recognized for improving the quality, efficacy, and/or cost-effectiveness of diagnosis, treatment and care in the field of medicine. Winners will be named March 17, 2015. The BHW Group out of Austin, TX provided the web application and technical services for the development of CareCycle Navigator.
