San Diego Eldercare Agency Announces "Care to Home" Service for Smooth Transition from Hospital to Home

Care to Home assists individuals who are unable to drive themselves home from healthcare facilities and who require additional support in settling back into their home routines after a hospital stay of any duration.
In addition to transport services, a trained and insured Golden Care caregiver helps the discharged patient to get settled in his or her home upon return. Golden Care’s Care to Home package is $60.00 and is available to individuals who have been discharged from hospitals and skilled nursing facilities in the San Diego area. The Care to Home package includes transportation home and three hours of companion care from a licensed caregiver who is a member of the Golden Care team.
