Health Data Intelligence (HDI) is pleased to announce the introduction of the Preventive Care Dashboard into its premier CarePlan Dashboard Suite. The Preventive Care Dashboard is a critical tool in the hands of clinicians, hospital administrators, clinical nurse managers, and other hospital staff for tracking and monitoring compliance with immunizations for influenza and pneumonia for hospital patients for whom such immunizations are recommended.

The Preventive Care Dashboard tracks hospital performance on two key metrics: Immunization for Influenza and Immunization for Pneumonia. The influenza prevention measure covers acute care hospitalized inpatients who have been screened for seasonal influenza immunization status and were vaccinated prior to discharge if indicated. The pneumonia prevention measure covers acute care hospitalized inpatients who are considered high risk and have been screened for receipt of the pneumococcal polysaccharide vaccine and who were vaccinated prior to discharge if indicated. In both cases, the screening and intervention of vaccine administration when indicated at each hospital is tracked and measured against state and national benchmarks for compliance.

Navigation within the Preventive Care Dashboard occurs via tabs, with graphical and tabular views for each time period selected, tooltips that provide rich metadata when a user hovers over any data point, filtering capabilities down to the individual hospitals or health system, CMS measure codes to help users uniquely identify each metric, and an About page that provides detailed metric definitions, external links, and other useful information.

Available for benchmarking on the Preventive Care Dashboard are state averages and the national average for each immunization measure as well as each hospital’s internal targets and organizational performance goals.
