“HRS’ innovative way to monitor patients with an increased risk of needing costly emergency room care or additional admissions into the hospital will add an important product to the services offered by KVNA,” according to Michael Puskarich, CEO of KVNA.
Kenosha, WI (PRWEB) April 10, 2017
Kenosha VNA, a 4 ½-Star home care provider, announces their partnership with Health Recovery Solutions (HRS) to provide patient engagement and monitoring platform in Wisconsin.
The agency will be the first in the state of WI to leverage Bluetooth connected blood pressure monitors, pulse oximeters and scales to provide improved care and reduce readmission rates for their patients with Congestive Heart Failure (CHF). Each patient will receive a customized care plan, will be able to have their symptoms and vitals monitored in real time, and contact their clinicians and family members through video visits, phone calls, and text messages using the HRS tablet. Kenosha VNA will target other high risk populations including patients with COPD and Diabetes.
“HRS’s innovative way to monitor patients with an increased risk of needing costly emergency room care or additional admissions into the hospital will add an important product to the services offered by KVNA,” according to Michael Puskarich, CEO of KVNA. “This partnership continues the long legacy of KVNA’s ability to bring innovation, individualized care and great outcomes to the patients of Southeastern Wisconsin to reduce the overall costs of health care.”
HRS focuses on patient engagement and provides resources for patients to learn how to better manage their own care. The tablet software allows for easy access to educational videos, quizzes, care plans, surveys, and other useful documents that are all specific to their diagnosis. Additionally, family members and other caregivers can help monitor and engage patients through the Caregiver Connect mobile app that allows them to track compliance.
“We’re thrilled to be partnering with Kenosha VNA to help provide innovative remote patient monitoring.” said Jarrett Bauer, CEO of Health Recovery Solutions. “Patient engagement is such a crucial element in remote patient care, and Kenosha VNA will be an amazing partner to help reduce readmission rates and improve health outcomes in Wisconsin.”
Kenosha VNA is rated 4 ½ stars in patient outcomes by the Centers for Medicare and Medicaid Services, accredited by the Joint Commission.
