National Transitions of Care Coalition Adds Partners Council Member
Washington, DC (PRWEB) December 09, 2014 -- The National Transitions of Care Coalition (NTOCC), an organization dedicated to improving patient transitions throughout the healthcare system, is proud to announce the addition of a new member to its Partners Council. Global Transitional Care is a healthcare service company that provides short-term care services to patients transitioning from hospital, or in-patient care facility, to home.
“NTOCC is thrilled to welcome Global Transitional Care as the newest member of our rapidly growing industry Partners Council,” said NTOCC Executive Director, Cheri Lattimer. “GTC’s dedicated focus on innovative services that specifically improve transitional care from the hospital/inpatient facility to home, and help ensuring seamless post-discharge care for patients/caregivers and the entire healthcare team, provides a natural alignment with NTOCC’s overarching mission of improving patient transitions and outcomes. The GTC leadership team also brings a diversity of experience from across the healthcare industry, and a critical understanding of the importance of continuity of care at one of the most problematic transition points in a patient’s journey through the health care delivery system.”
“NTOCC is very excited to partner with Global Transitional Care as we continue to expand the reach and dimension of our valued industry partners team, which supports the NTOCC mission in so many ways – including thought leadership and the creation and delivery of innovative tools, resources, and information that assist the entire NTOCC community in improving transitions of care for patients, caregivers, and health care providers of all kinds,” Lattimer added.
The Partners Council is a select group of industry partners representing various segments of health care and support industries, and brings together unique experiences, skill sets, products and solutions that allow NTOCC to further the mission of advancing solutions to challenges involving transitions of care. The Partners Council considers companies in the pharmaceutical, acute care delivery, managed care, retail pharmacy, medical device manufacturing, health care service and information technology sectors for membership.
“GTC is excited at the opportunity to partner with NTOCC,” said Hisham El-Bayar, President & CMO of Global Transitional Care. “NTOCC plays a crucial role in bringing like-minded organizations together to drive innovation through collaboration in this important area of healthcare delivery,” said a GTC representative. “In line with NTOCC’S vision, GTC was founded with the goal of bringing quality of care to the next level, while at the same time providing cost benefit and efficiencies to healthcare institutions and providers. GTC is looking forward to being a member of the NTOCC partner’s council and playing a part in setting the standard of care for Transitions of Care Management.”
Companies that are invited to join the Partners Council enjoy various avenues for collaboration and dialogue within the NTOCC community, the ability to work directly with industry peers in crafting strategies and solutions to improve patient transitions, and enhanced opportunities to partner directly with NTOCC in exploring TOC solutions. About Global Transitional Care
Global Transitional Care (GTC) is the first third-party organization dedicated to providing complete transitional care services.
