Symphony's ability to mirror the clinical workflow using smart checklists and the ability to support a patient as they transition through a system of healthcare has been a tremendous satisfier for the clinical teams and the administrative support staff.
(PRWEB) October 01, 2013
Performance Clinical Systems announced today at the Health 2.0 Conference the commercial availability of the latest release their cloud-based groupware application for clinical workflows, Symphony 4.0. One of the company’s key clients, Holston Medical Group (HMG) in Kingsport, Tennessee, implemented the Symphony update as an essential component of their Accountable Care Organization's (ACO) quality and performance strategies.
Symphony 4.0 works in unison with traditional EMRs and other enterprise HIT systems, but offers a distinctly different and incremental set of functionality not found in any of those systems. Using a patented "dynamic checklist" system, Symphony provides decision support and performance documentation for virtually any patient encounter. HMG is using the platform for transitions in care handoffs from inpatient hospitalists to primary care practices to better manage post-hospitalization care plans. They are also using Symphony across all clinical settings to achieve and document 29 of the 33 quality and performance measures required in the Medicare Shared Savings Program.
"Symphony enables our ACO teams to drive compliance and better outcomes by moving through clinical encounters systematically and consistently, then alerting downstream team members of specific outreach opportunities," said HMG's President & CEO, Scott Fowler, JD, MD. "Now any member of the care team can view prior patient encounters that directly impact the patient’s care at the point of service, regardless of place of service and EMR platform."
Additionally, Symphony 4.0 provides a patient-centric, real-time layer of information that extends beyond the basic clinical values captured in the EMR. Vital information about patient's attitude regarding their condition, their belief on how well they can manage a condition, their home environment, caregiver situation, and any behavioral factors are all documented within Symphony and made available throughout the clinical workflow.
"We are using Symphony in our Wellness Initiatives covering both traditional Medicare and Medicare Advantage programs, systematically capturing more relevant and measurable patient information in a more efficient manner than through the previous process," said Wendy Oberdick, MD, HMG's Patient Centered Medical Home Medical Director. "Symphony's ability to mirror the clinical workflow using smart checklists and the ability to support a patient as they transition through a system of healthcare has been a tremendous satisfier for both the clinical teams and the administrative support staff at HMG."
Performance Clinical Systems is presenting the latest update to Symphony at the Sixth Annual Health 2.0 Fall Conference today at the Santa Clara Convention Center in Silicon Valley.
"It has been a delight working with the highly innovative and forward-thinking team at HMG," commented Performance Clinical Systems’ co-founder and chief scientist, Christopher Johnson, MD. A featured speaker today at Health 2.0, Dr. Johnson added, "They quickly identified needs beyond what their traditional EMRs could provide as they focused on coordinating care across facilities, other partner organizations and payer contracts. By inserting Symphony into an expanding set of clinical workflows, HMG is positioning themselves as a leader in patient-centric care as measured by goal achievement for quality and performance."

About Performance Clinical Systems
Performance Clinical Systems is a privately held healthcare information technology company based in San Francisco, California and Boston, Massachusetts. Founded in 2004 by two physicians, its core groupware application, Symphony™, inserts evidence-based interactive clinical checklists into clinical work-flow directly at the point of care, providing care coordination support to front-line clinicians of all types, while simultaneously gathering essential data for advanced analytics and quality improvement.
The company’s solutions are attractive to performance-oriented health care providers and payers because they are quickly embraced by clinicians, deployed rapidly (within 60 days) and work alongside or as a layer on traditional enterprise HIT systems. Most importantly, Symphony™ contributes significantly to a provider’s ability to handle shifts in payment models, such as Accountable Care Organizations (ACO’s) and Patient-centered Medical Homes (PCMH), which reward better care at lower cost.
