US Federal Contractor Registration: Department of Agriculture Releases Contract for Quarterly Wellness Services and Seminars on FebBizOpps
Washington D.C. (PRWEB) November 10, 2014 -- US Federal Contractor Registration is reporting a contract released on FedBizOpps (FBO) by the Department of Agriculture for Quarterly Wellness Services and Seminars on November 6th, 2014. The Quarterly Wellness Services and Seminars contract was originally released on October 20th, 2014. However, Business and Industry Student Trainee and Primary Point of Contact Renee Nicole Jones amended the original contract to add new terms. The modification to the Quarterly Wellness Services and Seminars solicitation resolved a discrepancy between the due date listed in the RFQ/SOW attachment and the due date listed on the actual solicitation. The Quarterly Wellness Services and Seminars is a Total Small Business contract, which means only small business government contractors are permitted to bid on this solicitation. This posting solicits proposals for Quarterly Health Assessments at the agency location in Washington, DC. The assessments will each be in three parts, with the first covering blood work, the second covering fitness testing and the third will be providing a printed report for use in counseling individual members (the report will be given to the member). Members who decline the counseling session will have the option of having the report mailed to them. In addition to this quarterly fitness assessment, an annual fitness fair is to be held in August/September to take place at both locations (not simultaneously). Subjects should include, but are not limited to; Glaucoma testing, lung capacity, percentage of body fat, blood pressure, osteoporosis testing, and skin damage testing. Interested parties should read the attached Statement of Work and provide costs per individual for CLINs 001-004 and optional CLIN 007 and provide a price per seminar for CLIN 005. For CLIN 007 and optional CLIN 008 should provide a total price. Please provide prices for the base year and four additional option years.
