Re-engineering the currently disjointed processes for managing medications to produce an integrated, patient-centered approach is a powerful lever for moving the needle on health outcomes and driving down costs.

As healthcare moves toward patient-centricity and value-based care, a more comprehensive model is needed to improve medication management and care collaboration across health care silos and stakeholders.

To address this industry need, speakers from Point-of-Care Partners and the Cleveland Clinic Florida will introduce eMedication Management (#eMedMgt) at the HIMSS14 Annual Conference and Exhibition in Orlando, Fla. The session, “Improving Care Transitions with IT-enabled Medication Management," will be held Wednesday, February 26, at 8:30 AM in Room 311E of the Orange County Convention Center.

The eMedication Management model leverages the health IT infrastructure that exists today and is designed to improve medication adherence, among the most costly challenges facing health care today. Patients’ noncompliance with medication regimens costs an estimated $289 billion a year—much of that due to expensive hospital readmissions, avoidable emergency room visits and unnecessary deaths.

  • Facilitate prescribing medications that take into account a patient’s medical history (such as test results and comorbidities)

  • Reconcile and manage medications for patients as they transition among inpatient and outpatient settings and multiple types of providers
  • Engage patients in their care to better understand their medication regimens and boost compliance. This also helps providers meet meaningful use (MU) requirements
  • Assess outcomes and report on results to meet quality and payment targets, both at the ACO and federal levels
  • Help payers optimize their health plans’ medical loss ratios, as eMedication Management represents a health IT investment to improve quality
