Enclara Health and Hospice Pharmacia Unveil Updated Name and Logo at National Hospice and Palliative Care Conference

The new company name and logo has been unveiled in Booth #700 at the 15th Clinical Team Conference and Pediatric Intensive. The trade show and conference produced by the National Hospice and Palliative Care Organization (NHPCO), in cooperation with the Tennessee Hospice Organization, is taking place this week at the Gaylord Opryland Resort and Convention Center in Nashville, Tennessee.

On Tuesday, October 28, 2014 from 4:00 - 5:00 pm in the Presidential Ballroom, Terri Maxwell, PhD, APRN and Loren Brook, PharmD, both of Enclara, together with Judi Lund Person, MPH and Jennifer Kennedy, MA, BSN, will present on the Guidance and Support for Hospices under CMS' Updated Medicare D Rules.
The Learning Objectives for the session include summarizing CMS' policy on handling Part D payment for beneficiaries enrolled in hospice; describing best practices for determining and documenting relatedness and demonstrating strategies for identifying and communicating about medication discontinuation or other medication changes with patients, caregivers and prescribers.
