Oculus Health Releases Disease Management Modules to their Chronic Care Management Suite

These condition specific modules will facilitate the most targeted and effective management of chronic conditions such as diabetes, heart disease, hypertension, asthma, and stroke. The product, Oculus Trackers™, delivers an evidence based individualized care plan for each condition, and assists providers with customizing patient care plans. According to Chief Medical Officer, Kishori Somyreddy, M.D., this will ensure that the most proven disease mechanisms are effectively delivered to patients, and that they are able to follow and report their care plan adherence in a consistent manner. Dr. Somyreddy led the team of clinicians in the development of the product.
