Altruista Health Selected to Support Significant Population Health Initiatives in Tennessee and New York
Reston, VA (PRWEB) March 02, 2016 -- Altruista Health, Inc., an innovative provider of population health management technology solutions for health plans and health care providers, today announced it has already signed several agreements in 2016 to support new or expanded population health management initiatives. Collectively these new projects cover more than 3.5 million people in several states, including Tennessee, New York, Massachusetts and Iowa, bringing the total number of lives managed on the GuidingCare platform to more than 11 million.
The new agreements and initiatives include:
• A care coordination technology platform and related services to support the State of Tennessee Health Care Innovation Initiative, a broad coalition of public and private stakeholders that will implement complementary payment and delivery system reform initiatives. Altruista Health’s GuidingCare system will be used statewide by multiple payers and providers as a shared platform for gaps-in-care identification, care planning, care coordination and other workflow management.
• Comprehensive population health management for a large Medicaid health plan in New York. Altruista Health will provide an integrated system for care management, disease management, utilization management and care team coordination for nearly 1.5 million government and commercial plan enrollees.
• Continued statewide expansion to support Medicaid, Medicare and CHIP programs for one of the premier nationwide health plans. Altruista Health currently supports care management, disease management, care team coordination, member engagement and social support initiatives for this client in 28 states, and will be expanding to several more throughout 2016.
• A care management and coordination platform for a leading behavioral health provider organization in Oregon. GuidingCare will be leveraged by networks of community health clinics and practitioners providing mental health services through a Coordinated Care Organization (CCO).
