Inland Empire Health Plan Selects Mirixa for its Innovative Pharmacy Home Program
Reston, Virginia (PRWEB) November 30, 2016 -- Mirixa Corporation, the leading provider of medication-related services to health plans and PBMs, announced that Inland Empire Health Plan (IEHP) has selected Mirixa Corporation and the MirixaPro platform to administer medication therapy management services for the IEHP Pharmacy Home Program.
The IEHP Pharmacy Home program is one of the first large-scale pharmacy Pay-for-Performance (P4P) programs in the United States. The IEHP Pharmacy Home program teams with local pharmacies to help IEHP members get needed care. This program focuses on care for diabetes, high blood pressure, high cholesterol, and asthma. IEHP-qualified in-network pharmacies help eligible members better manage their medicines. This comprehensive service puts information about members’ medicines and related care in one place. Based on the “medical home” model, the Pharmacy Home program places the member at the center of care.
The MirixaPro platform plays a key role in facilitating the IEHP Pharmacy Home program. Mirixa network pharmacies provide ongoing medication therapy management services, including safety alerts, omission and adherence care gaps, and care coordination where high-risk members are followed for a period of time to achieve optimal clinical outcomes.
Benefits of the Pharmacy Home program include:
- Customized person-to-person consultation – pharmacists will discuss treatment goals with members and help them to achieve these goals by the end of each program year
- Comprehensive medicine review – pharmacists will review how IEHP members’ prescriptions and over-the-counter medicines may affect each other
- Guidance on taking medicines the right way – IEHP members will receive details on how to take their medicines correctly and will get recommendations on avoiding adverse side effects
