Somerville, NJ (PRWEB) October 17, 2017 -- Community Visiting Nurse Association, a 4-Star home health agency for both patient quality of care and satisfaction in New Jersey, continues its excellence in care using innovative technology. Last quarter, patients on the telehealth program had a 30-day readmission rate of 7.89%. A dramatic difference from New Jersey’s average readmission rate for such patients which currently stands at 16.9% while the national average is 16.7%.

For the first six months of 2017, patients placed on Health Recovery Solutions (HRS) telehealth software had high adherence rates for taking their blood pressure (89.9%), pulse oximeter readings (82%), and weight (83.3%) every single day. These high-risk patients with several chronic diseases are now coming home with greater and greater needs presenting many unique challenges for home health agencies; Community VNA has incorporated a successful innovative telehealth program to help those high-risk patients under their care. “The use of technology, specifically telehealth that promotes continuity of care, provides greater understanding of factors impacting care and overall improvement of clinical outcomes.
