Oak Street Health Appoints David Buchanan M.D., M.S. as Executive Medical Director, Illinois
CHICAGO (PRWEB) January 18, 2018 -- Oak Street Health, a network of health centers providing a new model of value-based primary care for Medicare patients, has named David Buchanan M.D., M.S. as Executive Medical Director for Illinois. Dr. Buchanan will be responsible for leading the primary care, APN and complex care teams in Illinois. Additionally, he will advance critical clinical initiatives that support safety, population health, and quality operations across the organization.
Dr. Buchanan is an accomplished clinician and leader, with an impressive track record of success for scaling organizations and championing strategic initiatives. Most recently he served as Chief Clinical Officer of Erie Family Health Center. In that role, he more than doubled the size of the practice, recruited and led more than 200 medical, behavioral health, and oral health providers, and designed an innovative care model including care management and the population health program. He chaired multiple boards and Clinical Committees, including for a Coordinated Care Entity, an Accountable Care Organization and a Health Center Controlled Network focused on Health IT.
Prior to his time at Erie, Dr. Buchanan held multiple positions at the Cook County Health and Hospital System, including Head of the Section of Social Medicine and Director of Medical Student Programs at Cook County Hospital.
