DSRIP PPS Alliance For Better Health Partners With Unite Us to Improve Health Outcomes in Upstate New York
NEW YORK (PRWEB) January 23, 2018 -- Unite Us, a Veteran-owned technology company, announced that it has partnered with Alliance for Better Health (Alliance) as the software provider that will power Alliance’s coordinated community-wide care network.
Alliance’s partners will be delivering care as part of an integrated and accountable network of both healthcare and social service providers. Providers will be able to electronically connect patients to community partners, and report on every outcome achieved inside and outside the clinical setting. This empowers Alliance and its partners to directly address patients’ social determinants of health, ensuring patients are at the center of care, while tracking their health journey across the community.

The Unite Us platform will allow Alliance’s clinical partners throughout Albany, Fulton, Montgomery, Rensselaer, Saratoga, and Schenectady counties to connect in real time with social service providers such as housing and workforce development agencies, behavioral health providers, and food pantries.
All partners across the network will be centrally connected to each other, with the ability to:
Electronically connect patients to both clinical and social service providers.
- Track patient progress & receive automated feedback from partners to ensure care and services are received.
- Collaborate with community-wide “care teams” ensuring providers are on the same page regarding their shared patients’ progress.
- Collect data including structured patient outcomes to measure the network’s impact, including time to service and effectiveness of care.
