Fit4D established a partnership with Humana in April 2017 to provide its diabetes coaching solution to members in North and Central Florida. This new expansion is part of Humana’s Bold Goal initiative to make the Broward community 20% healthier by 2020, and will now serve more diabetes members in Florida.
There are over 30 million people with diabetes in the United States, with a disproportionate amount affecting high-risk populations, including seniors and low-income households. Fit4D successfully combines two important elements: One-on-one support from Certified Diabetes Educators (CDEs), clinicians who specialize in motivating and teaching people with diabetes how to manage their condition, along with technology to scale the reach of CDEs so that greater numbers of patients can have convenient access to specialized education and care. With the increasing social and economic disparities in healthcare, this is particularly important, as Fit4D reaches underserved populations who would not ordinarily be able to access personalized care from a diabetes clinical expert.
