Cadalys Care, Informed by MCG for Post-Acute Care, is designed to enhance communication and streamline workflow across organizations, leading to more efficient use of medical resources and improved patient satisfaction and health outcomes.

Cadalys Care, Informed by MCG, is available on Salesforce Health Cloud and makes evidence-based care guidance readily accessible to those invested in a patient’s health journey. This means multiple providers can get a complete view of patients in one location from within Health Cloud and access information across the care journey, including everything from current conditions to medications. A single platform for information sharing enables smarter care decisions and better care coordination, along with the opportunity to engage more deeply with patients. Care team members can access daily task/patient prioritization dashboards, interact with other care managers via Chatter, quickly identify patient progress using a timeline view of events, and customize dashboards for high-level views of relevant information. Because this is all built on the Salesforce Platform, the application can be easily customized to extend the workflow, provide automation, and leverage other custom or AppExchange applications in a unified way.

The app incorporates MCG’s Home Care and Recovery Facility Care content, which provides comprehensive decision support to help clinicians practice to a standard of care. Post-Acute Care guidance offers evidence-based criteria to select the appropriate level of care following a hospitalization, such as Skilled Nursing, Inpatient Rehabilitation, or Home Care, to enable optimal transitions and reduce avoidable complications that could lead to readmissions. This solution empowers clinicians to make informed decisions that optimize the level-of-care and ensure an effective path to patient recovery.

Once a patient is transitioned to the appropriate care setting, MCG’s proprietary benchmarking information provides a simple way to monitor patient progress and set customizable targets around skilled nursing length of stay, discharge date, and number of home care visits. Cadalys Care presents MCG Benchmarks and Data in an easy-to-use, interactive format that allows for quick comparisons against national and regional statistics for commercial and Medicare populations (based on large national claims databases).

An early adopter and contributor in the development of this solution is Access Innovations. While piloting post-acute care redesign programs, they recognized the need for an integrated solution that provided access to evidence-based content and allowed for coordination of care around the patient.
