RESTON, Va., July 12, 2018 /PRNewswire-PRWeb/ -- Altruista Health, Inc. today announced that Commonwealth Care Alliance® (CCA) has selected GuidingCare® as its new care management platform for its member and patient services. CCA, a not-for-profit, community-based healthcare organization in Massachusetts, serves individuals who are dually eligible for Medicaid and Medicare with complex medical, behavioral and social needs. Research shows that these vulnerable populations benefit from highly coordinated, patient-centered care that addresses the total health picture.
CCA will use this member-centered platform to deliver high-quality care experiences and outcomes, as well as to increase efficiency, care partner communication, reporting and automation, and to support long-term growth. The platform will also offer CCA's field clinicians improved access to member and patient information through a mobile clinician app.
GuidingCare is used by health plans and provider organizations to streamline care management workflows, facilitate coordination among clinical, behavioral and community resources, accelerate quality improvement and promote engagement.
Serving more than 27,000 health plan members with significant healthcare needs, CCA will use GuidingCare to automate the complex workflows necessary to manage this population. The platform's data analytics and decision support engine will assist CCA in identifying, stratifying and predicting risk among members, informing preventive care measures, and targeting intensive interventions accordingly.
"After a thorough market review, we chose Altruista's GuidingCare software because it offers configurable options and actionable solutions based upon its data reporting and analytical tools," said Christopher D. Palmieri, President, and CEO of CCA. "It will support our care management team's collaborative work to improve member outcomes in several ways, including population health reporting and deployment of care delivery and management. It will also help CCA to maximize operational efficiency and membership growth long-term while ensuring we fulfill our mission of providing the best possible care, tailored individually to the patients and members we serve."
"We are excited to support CCA, which has been recognized for its top ratings by consumers in quality and access," said Ashish Kachru, Altruista Health Chief Executive Officer. "Our organizational missions are perfectly aligned. Working together, we will help the most medically complex members of these programs improve health outcomes at the same time we mitigate risk and reduce avoidable expenses."
